It’s a tender time. A time when true compassion strengthened by physical, emotional and spiritual support can bring a balm of comfort to those with life-limiting illness. Family and dear friends are relieved of caregiving and can focus on time together. At the heart of Rehab and Health Center’s mission and vision is the belief that each person deserves to live each day to its fullest, and each family deserves to be fully supported and informed. Our dedicated caregivers are deeply devoted to this belief and enriched by those we serve.

The patient and family are included in the care plan. Support is given based on the patient’s wishes and family’s needs. Team members provide palliative care, including symptom and pain management, with interdisciplinary hospice teams providing case management, social work and chaplain support.
