To ensure that each resident has exactly the right amount of care we do a functional assessment before admission and at regular intervals afterward.
The charge for the initial assessment is nonrefundable. There is no charge for the periodic follow-up assessments.
Admission Requirements
- Completed application
- Appointment for functional assessment and apartment assignment
- Chest x-ray report (within 90 days) stating resident does not have any active disease
- Prescription from resident?s physician for an annual TB test (PPD)
- Letter from physician stating resident is free from all communicable diseases
- Copy of resident?s insurance cards
- Medication and diagnosis form filled out and signed by resident’s physician
- Copy of Medical Advocacy or Durable Power of Attorney if applicable
